Regform

Fill out the form below to register for any part of Tara’s workshop October 19-20, 2024. This completed form is due: Sept. 18, 2024 to secure registration.

If you are not able to pay online, please contact me and we will arrange for payment by check.

Payment and Cancellation Policy: Fees include workshop instruction. Fees do not include any workshop supplies, lodging, food, airfare, or any other expenses.

Cancellation and Refund Policy: Tuition is refundable less a $75 administrative fee that will be retained for all cancellations.  There is considerable work involved in registering, accounting for, and corresponding with students to make this charge valid.  For cancellations less than 21 days before the workshop begins, tuition is NON-REFUNDABLE unless a waitlist participant takes your place.  No refunds will be given for no-shows or withdrawals from a workshop that has begun, regardless of the reason. In the rare case a workshop is canceled due to the illness of the instructor or due to insufficient enrollment, weather, etc., the student may, at the discretion of CMS, receive a full refund.  This is to be determined 1-2 weeks prior to the first day of the workshop.  We recommend you purchase refundable airline tickets or purchase travel insurance to cover the cost in case of a workshop cancellation.

Please Read and Sign this Agreement/Hold Harmless: Your participation in a workshop at Carrie Moore Studios is appreciated, and we will work hard to ensure a great experience. However, there are situations beyond our control, and each student artist needs to be responsible for their own actions and precautions when working with art supplies, working in or out of doors, and traveling. Sponsoring workshop entities have taken much care in providing suitable places to paint and study. Be aware of your equipment at all times, as we cannot be responsible for loss or damage.

Therefore, by printing my name in the below box, I, the undersigned, agree to release from responsibility and hold harmless Carrie Moore, Tara Will, and any hosting instructor and all of their employees and collective heirs for any losses, delays, damage, or injury I may incur or sustain as a result of accident or negligence beyond their control, and I accept full responsibility for all the rest inherent in attending an indoor/outdoor painting workshop and any form of travel.  I will not make the artwork I created in this workshop available for sale unless I have written approval from Tara Will, instructor.
